If money is not a factor and you want the highest quality, OPS-CORE AMP headsets are fire. They will also light your wallet on fire...

If you want something real and high end, get a set of Peltor Comtacs or a set of MSA Sordins. Be warned, they are pricey and they have a number of different models that may or may not have comms capability. Sordins are also a pain in the ass to helmet mount.

If you want something real, decent quality and not too expensive, get a set of Howard Leight Impact Sports. To upgrade it to be more comfortable, get a set of Noisefighter gel cups for it.

If you want something that's airsoft grade and kind of hit or miss, the Earmor sets are popular. I have seen a few sets break or stop working but that's what you get with airsoft grade gear.

If you're on a budget, there's no reason to not just buy Howard Leight Impact Sports, iirc I bought mine 2 years ago for between $60-80. They're budget friendly, you can mod them to have noisefighter gel cups and attach to Peltor Arc adapters, etc.

Depends on what your headset and plugs are. I'm using Comtacs, Baofengs, and Earmor PTTs (the Earmor PTTs kind of suck ass but they're pretty cheap and they work).

If you're using Howard Leights, get a shoulder mic with a 3.5mm aux jack and run your headset to it with an audio cable (this is the typical budget comms setup).

Code Red stuff is typically regarded fairly well (no personal experience with them though).
